Disconnection of arterial collateral as the cause of lower leg amputation after conquassation caused by petrol tiller: A case report

Introduction. Leg conquassation caused by petrol tiller is one of the most severe injuries in bone and joint traumatology. Firm strokes sharp blades produce strong force that easily damages both soft tissues bones. Since tillers are used soil processing, wounds highly contaminated with dirt fertilizers, hence anaerobic spore-forming bacilli, such as tetanus gas gangrene pathogens. Case report. This paper presents treatment a 69 years old man chronic arterial insufficiency lower extremities who suffered injury leg (IIIB open tibial fracture according to Gustillo) while performing agricultural work. Due absence pulsations, Multislice CT angiography arteriography were performed. Arteriography injured confirmed occlusion anterior artery numerous stenotic lesions peroneal posterior arteries. Posterior was chronically occluded its distal part connected foot moderately developed collateral arteries which provided viability leg. Despite undertaken basic principles this serious (primary surgical wounds, external fixation, reconstruction tissue, antibiotic anti-tetanus prophylaxis) due infection ended amputation. Conclusion. amputation can be expected type cases extensive destruction tissue field existing elderly patients, even main blood vessels traumatic disconnection patients.
